All faculty and presenters must complete this disclosure form.

PowerPoint Presentations

All Presentations must include a Disclosure slide. Please list all disclosures and if none list "no disclosures."
This includes “co-authors” that may not attend as a presenter but contributed to content. 

A sample ICHS PowerPoint is available for your presentation - there is no requirement to use the template, but speakers are welcome to use them if desired. Click here for the template.

This event is under review for accreditation credits. As such, the presentations must be reviewed in advance for approval.

Please prepare and submit your presentation as follows:

• Use at least 24-point font for body text and at least 36-point font for headings and titles
• Adjust font size based on the amount of text, font type, and screen resolution.
• Please review slides with compatibility features.
• Keep slides clean: avoid adding too much information, especially graphs and experimental data on one single slide. When using images, please ensure a high quality, royalty free (or cited) image. Choose colors sparingly.
• Presentation files must be in .ppt and .pptx only and under 500 MB in size. The aspect ratio of your presentation must be 16:9 and to avoid compatibility problems, please do not use special characters in the name of the file (e.g. «, Ö, Ø, ñ, ε, ®, ý, }, {, etc.). 
• Duration and number of slides: when preparing your presentation, please take note of the time allocated to your presentation. In general, 2 - 2.5 slides per minute is a reliable guide to ensure that you do not exceed the allotted time.
